Percentage of all-out innings in limited-over games, and declarations in Tests etc

Is there any relevant record about the percentage of all-out innings in limited-over games (50, 20, or YB40)?
Is there any relevant record about the percentage of declarations in Tests and other First Class games? :D

You know I'd actually be really interested in a percentage-of-innings-declared over certain time periods historically; I suspect it'd make for a slightly more honest appraisal of whether bat is generally dominating ball or not than just simple average.
