Worst appeal of all time?

In the fair dinkum department, that lbw was absolutely ridiculous. Surely you couldn't live with yourself giving that out. Gatting's reaction was completely priceless.
That was by no means the worst decision that Shakeel Khan made in that match. There was a certain amount of history behind his umpiring and even some Pakistan cricket writers believed that it was deliberate cheating. Pakistan had always objected to David Constant umpiring their matches in England after some poor decisions he made that were very costly. England being England (smug as ****) ignored their request and allowed Constant to umpire. Then when England objected to Khan standing the Pakistan authorities rightly ignored their complaints. He then proceded to fire out England batsman as shown in that youtube clip. It lead to the Chris Broad refusing to leave the crease situation and was partly responsible for the frustration that lead to Gatting's row with Shakoor Rana. At the time it was generally considered that Shakoor Rana (RIP) was just an incompetent buffoon but that Shakeel Khan's decisions were so bad that it had to be deliberate cheating. Mike Gatting sacrificed a winning position in the final test on a point of principle and was then sold down the river by the MCC.
Australia were subjected to equally appalling umpiring the following winter. Pakistan had long since been calling for neutral umpires and these two series went a long way towards ensuring it happened.

The worst appeal and lbw decision I ever saw live was Alec Stewart being given out to a ~50mph ball from Sanath Jayasuriya that pitched ~50cm outside leg-stump. The fact that it pitched outside leg was (or should have been) blatantly obvious before it even hit Stewart on the pad, yet Peter Manuel saw fit to give it out.

However, there will have been many as bad.

The only one I've seen worse than that was David Constant giving Alan Knott out LBW to Malcolm Marshall in 1980. He bowled a bouncer around the wicket which pitched well outside the leg stump and hit Knott on the elbow as he ducked, and would have cleared the stumps by two feet. Jim Laker kindly described it as "astonishing".

That would totally work, but it would still result in Waugh being pissed off, one of the very few people to get into Waugh's head and actively irritate him. I rate Ganguly highly as a captain just for that.

Showing up late to the toss just to piss Waugh off was childish, but so, so priceless.

Pretty much spot on Constant was a constant pain in Pakistan's butt though have to disagree on the aus series the favouritisim to the home team was no different to anyother series was played during that time. Also during Imran's tenure as Pakistan captain most home series played under his captaincy was played with two neutral umpires.
